*What is the Maestro Symptom Survey?
The Maestro Symptom Survey allows patients to easily and safely provide confidential and important medical information which will help Dr. Pickens determine the best treatment and care.
Symptom Survey Nutritional Analysis:
- Evaluates patient Symptom Survey forms and suggests Standard Process and MediHerb supplementation based on a modified symptom group analysis, actual clinical experience, and traditional application of whole-food nutrition and whole-extract herbal therapies.
- Evaluates additional test results - Blood Pressure and Raglan test results.
- Records Five Main Complaints of patient for each Symptom Survey Form.
- Prints concise Clinician Reports detailing body system groups needing support.
- Manages patient information database storing entered forms for future comparison and review.
All patient accounts are fully secured with 128 bit encryption.
How do I take the survey Free of charge?
Step 1. Contact Sylvia Pickens at 888.289.1697 for complete survey information. Or e-mail at medicalwonders@keptprivate.com.
Step 2. Download the general "Patient Information" form.

The Patient Information form must be returned to Dr. James E. Pickens before the Maestro Patient Survey can be taken.
Step 3. Make sure you have the password provided by Sylvia. The account number you will need is A002930. Go to www.DoMySurvey.com, enter the information required to take the survey.
